Register onlineComplete the form below. It takes a few minutes and tells the athletic office who is playing what.

Complete medical clearanceNew Jersey requires a Pre-Participation Physical Evaluation completed and signed by a physician, plus a Health History Update Form. Details below.
A student-athlete cannot practice or compete until medical clearance is on file. The physical evaluation must be completed by a licensed physician, advanced practice nurse or physician assistant. It cannot be completed online, and this website does not collect medical information.
Ask the athletic office for the current New Jersey Department of Education packet, or download it from the NJSIAA website, then return the completed and signed forms to the athletic office. If you would rather not send paperwork on paper, call (800) 354-6073 and the athletic office will give you a secure way to send it.
